General Information about the Manor is enclosed along
with the necessary forms. These must be completed
and returned in their entirety before a prospective
resident can be considered for admission. These forms
are:
- Application for residency
- Financial Statement on prospective resident (and
sponsor, if applicable), same to be updated annually.
- Verification of Financial Statement(s). (Examples:
income tax return, bank statements, investment statements),
etc. These must also be updated annually.
- DSS 3122 Medical Evaluation, completed by applicant's
licensed physician
- These forms are to be accompanied by a $25.00
nonrefundable fee, payable to Westminster Manor.
After all the application documents have been received
and reviewed, the prospective resident and the person
responsible for his/her finances (son, daughter, sponsor,
etc.) will be invited for an interview with Westminster
administrative staff and an Admissions Committee member.
Following the interview, applicants and/or representatives
will be notified of the Admission Committee's decision
within one week by mail or phone.
If accepted, the prospective resident will be placed
on the Active Waiting List and notified of vacancies
in order of acceptance. If applicant refuses following
second offer, he/she will be placed at the bottom
of the list. Applicants will be placed on the Inactive
List upon request. If individuals on the Inactive
List for more than one year request to be on the Active
List, they will be required to re-submit Numbers two
and four above. A re-interview will take place if
desired by either party. Should any questions arise
during the admissions process, the Main Office will
be happy to assist you.
